Add ACE Medical to an Arma Reforger server

ACE Medical, by the ACE team, changes how injuries work in Arma Reforger. Find the right mod and ID, add it with ACE Core, and see what players notice.

Updated , checked on Arma Reforger 1.8.0.13

The ACE team’s medical mod for Arma Reforger is called ACE Medical Core. Add it together with ACE Core, which it needs, and restart the server. Players then fall unconscious instead of dying outright, feel pain effects, and need a medic to bring them back.

Which mod to add

The ACE team publishes two sets on the Workshop. Search for ACE Medical Core and you will see both. The author on each is acemod.

SetModIDVersionBuilt for
ReleaseACE Medical Core60C4C12DAE90727B1.4.31.7.0.41
ReleaseACE Core (needed by the one above)60C4CE4888FF46211.4.41.7.0.41
DevACE Medical Core Dev65860797892784131.5.361.8.0.10
DevACE Core Dev (needed by the one above)65AD7D0D9941A3801.5.391.8.0.10

Use one set, not both. Pick the Release set, but its page says it was built for an older game version (1.7.0.41), so test it before an op. The Dev set is the test version, tagged TESTDEV on the Workshop. The “built for” column is what the Workshop lists. The current stable game is 1.8.0.13. Not checked on 1.8 as of 12 October 2026 for either set.

Other authors publish patches with “ACE Medical” in their names, such as compatibility mods for other scenarios. They are not the ACE team’s mod. Check the author is acemod.

What players notice

The ACE documentation describes ACE Medical Core like this:

  • Second chance. A player who would have been killed outright falls unconscious instead. Whether that happens is a chance, and it differs by body part.
  • Pain. Injuries cause pain screen effects. Morphine treats pain and no longer heals.
  • Epinephrine. It speeds up waking an unconscious patient.
  • Repositioning. You can put an unconscious patient on their back or in the recovery position.
  • Medical radial menu. Open it with Ctrl+H (the ACE default).
  • Medical kit. Depending on the settings, healing uses up supplies, and full healing needs a place such as an ambulance or a medical facility.

In the ArthOS panel

  1. Open the Mods tab, then the Workshop tab.
  2. Search for ACE Medical Core. Choose Add on the card by acemod. The panel also adds the mod it needs, ACE Core.
  3. Open On this server and check that both are on the list.
  4. Restart the server. A change to the mod list takes effect at the next restart. See restart your server.

In the results, ACE Medical Core (acemod, 1.4.3) is 139.1 KB and ACE Core (acemod, 1.4.4) is 14.5 MB. The Dev versions of both also appear, so check the name before you choose Add.

By hand

In config.json, add both mods to game.mods:

{ "modId": "60C4CE4888FF4621", "name": "ACE Core", "version": "1.4.4" },
{ "modId": "60C4C12DAE90727B", "name": "ACE Medical Core", "version": "1.4.3" }

Save and restart. Keep the file valid JSON.

Settings

The ACE team documents settings that go in the missionHeader of the server config, under m_ACE_Settings. That is game.gameProperties.missionHeader in config.json. One example from their page, where booleans are written as 1 or 0 and decimals need a decimal point:

"missionHeader": {
  "m_ACE_Settings": {
    "m_ACE_Medical_Core": {
      "m_bBleedOutForPlayersEnabled": 1,
      "m_fBleedingRateScale": 1.0
    }
  }
}

Edit these in Files (config.json), or in Config Editor with Edit the whole file as text. There is no row for them in Config Editor, and a rotation entry does not change them. Keep the file valid JSON. If your server already has a missionHeader, add m_ACE_Settings inside it. Do not make a second one. The full list of settings is on the ACE page linked at the top.

If it does not work

  • The server will not start. Check that ACE Core is on the list as well, and that you did not mix the Release and Dev sets. Follow find the mod that stops your server.
  • Players still die at once. Second chance is a chance. By the ACE page’s defaults, a killing blow to the head gets no second chance. Read the settings above.
  • Errors in the log mention an ACE medical script. Another author published a fix for one such error, named “ACE_Medical_NULL pointer FIX”. It is not from the ACE team. Read its page first.
